Office Hours and Locations
Our offices are open from 8:00 a.m. until 5:00 p.m., Monday through Friday. Appointments may be scheduled with our staff during regular office hours. We will do our best to accomodate your schedule with the soonest possible appointment. Situations will and do arise that may result in cancellation of your appointment. We kindly ask that you give us 24 hours notice if you are unable to keep your appointment. We will attempt to contact you by telephone prior to your appointment.
When you arrive for your initial office visit, please have the following items with you:
- Completed patient information form
- All insurance cards and claim forms
- Driver's license or photo I.D.
- Medical records, X-rays, and other pertinent laboratory tests
- Medications (actual bottles or a list)
